DDD will receive the information you provided electronically through FOCUS. However, the application process does not stop there. Once you have submitted the online application, you must print, gather, sign and submit a hardcopy of the application and any requested assurance submittal documents. (Note: DDD is required to have your original signature on each document.)

After your electronic application has been submitted, you will receive a submittal confirmation email. You will then be required to complete Sections 9C through 9L of the application.
DDD has 30 days to screen all of your hardcopy documents. (If there is an issue, you will be notified and will have 30 days in which to respond.)
Upon approval of your application, DDD will schedule a Pre-Award Meeting with you, and a visit with the District Network Manager will be arranged.
When this process is complete, a contract will be issued.
If you are a non-therapy agency, you will be directed to the Office of Licensing, Certification, and Regulation (OLCR) to register your agency for a Home and Community Based Services certificate. OLCR will submit your HCBS certificate to the Arizona Health Care Cost Containment System (AHCCCS) for an AHCCCS Identification Number for you to use for billing.
DDD will then issue a “good to go” directive to the District Network Manager.
