Become A Qualified Vendor
The Request for Qualified Vendor Application (RFQVA) is the Division’s contract for Home and Community-Based Services (HCBS). For more information and a list of services currently being solicited, visit the Contract Information for Qualified Vendors web page and refer to the "Services Contracted under the RFQVA" section.

General Business Requirements
- Refer to the Arizona Corporation Commission on the Ten Steps to Starting a Business unless you plan to apply as a Sole Proprietor.
- Have an established internal system to ensure compliance with requirements for providers, employees, and contractors. This includes performing employee background checks, certification, service delivery, documentation, and billing for services.
- For applicants intending to apply for nursing and/or occupational, physical, or speech therapy services, obtain a National Provider Identifier (NPI).
- For applicants intending to apply for Nursing, Home Health Aid, and Licensed Health Aid, obtain a Home Health Agency License with the Arizona Department of Health Services.
