Caretakers Raising Children
Caretakers and grandparents who have court-ordered physical custody of a child for 30 consecutive days are eligible to receive child support services.
Some Child Support forms, documents and applications found in the Document Center may need to be notarized. If you need to submit one of the forms listed below, please be sure it is notarized before it's submitted. It may be submitted Electronically to [email protected] or through the Child Support Portal. Please follow your caseworker's instructions as some forms that require notarization may be generated and sent to you directly.
