File an Employment Service/Related Law Complaint

If you believe you have an employment service or an employment-related law problem, you may submit a complaint to DES and the Department of Labor. Law problems include, but aren’t limited to:

• Work mistreatment
• Wage problems
• Work benefits
• Work assistance issues and referral problems

Complainants have two years from the last act of alleged violation to submit a complaint.

How to Complete a Complaint

To file a complaint, you may complete one of the following two options:

• File a complaint online.
  

  OR

• Fill out the Complaint/Apparent Violation Form.
  

  Once the form is completed, you may submit it by any way below:

  • Email it to [email protected]
  • Print and submit the form in person to the nearest complaint specialist at a DES office or ARIZONA@WORK locations.
