Support Coordination

What is the role of the Support Coordinator?

A Support Coordinator has many roles. The most important role is to listen to the goals, choices, and vision of the member and, if they have one, their guardian, to develop a Person-Centered Service Plan (PCSP). Support Coordinators respect members’ interests, needs, culture, language, and belief system. Every member enrolled with DDD has a Support Coordinator.

What does a Support Coordinator do?

Support Coordinators:

  • Talk about supports and services and advocate on behalf of the member
  • Develop the Person-Centered Service Plan and update the plan with the planning team. The team includes the Support Coordinator, the DDD Member, and the guardian if the member has one. It may also include family/representatives, service providers, friends, advocates, and people who are important in the member’s life
  • Coordinate care, including physical health, behavioral health, Children’s Rehabilitative Services (CRS), and Long-Term Care Services, for eligible members
  • Provide information about community resources to meet the member’s needs
  • Identify and assist the member with individualized goals to reach the most self-fulfilling, goals to meet the member’s needs, desires, strengths, and preferences (e.g., in the areas of recreation, transportation, friendships, family, and other relationships).
  • Make sure the approved, funded services are in place and cost-effective
  • Confirm the member, guardian, and planning team know who to call if they have questions
  • Be a part of meetings with other agencies, like schools or behavioral health service agencies, that support the member
  • Provide support to solve problems
  • Make sure members know their rights and how to file a grievance

What if I am unable to contact my Support Coordinator or need additional help?

  • Call their Supervisor
  • If the Supervisor needs help, they have the support of the Area Program Manager
  • The Area Program Manager has the support of the District Program Manager
