DDD Eligibility for Children Ages 3 to 6 Years
How May a Child Be Eligible for DDD?
Have a significant delay in one or more developmental areas or be at risk for or have one of the following:
- Autism Spectrum Disorder
- Cerebral Palsy
- Down Syndrome
- Epilepsy
- Intellectual (Cognitive) Disability
- A 50% delay in one developmental area, or a 25% delay in two or more developmental areas
Developmental Areas include:
- Physical (fine, gross motor, vision, or hearing)
- Cognitive (learning, play skills, and problem-solving)
- Communication (understand language, communicate wants and needs)
- Self-Help (putting on clothes, feeding themselves, using the toilet)
- Social-Emotional (plays with others and toys, showing concern for others)
What Records Do I Need to Apply For DDD?
- Proof the family and child live in Arizona
- Proof of US Citizenship or Lawful Presence of the child
- A signed application by the parent/guardian
- Insurance Card, if you have one
- Medical Records or Developmental Evaluation that shows the child may be at risk for or have a developmental disability
What Happens After A Decision Is Made?
DDD and the family will work together to coordinate supports and resources for the child and family.
