DDD Eligibility for Children Birth to Three Years
It is suggested to first apply for the Arizona Early Intervention Program (AzEIP) if a child is suspected of having a disability or delay.

If a child is eligible for AzEIP, the Service Coordinator, with permission from the family will assist with the application to DDD. DDD will review records and determine if a child is also eligible for DDD.
Can a family apply to DDD before AzEIP?
Yes. DDD will also suggest the family contact Raising Special Kids and refer them to AzEIP. DDD helps with this process.
How may a child be eligible for DDD?
Have a significant delay in one or more developmental areas or be at risk for or have one of the following:
- Autism Spectrum Disorder
- Cerebral Palsy
- Down Syndrome
- Epilepsy
- Intellectual (Cognitive) Disability
What records do I need to apply for DDD?
- Proof the family and child live in Arizona
- Proof of US Citizenship or Lawful Presence of the child
- A signed application by the parent/guardian
- Insurance Card, if you have one
- Medical Records or Developmental Evaluation from a qualified professional that shows the child may be at risk for or have a developmental disability
What happens after a decision is made?
The AzEIP team, DDD, and family will work together to coordinate supports and resources for the child and family.
