Eligibility Redetermination is how DDD sees if an individual is still eligible for DDD services.
At the individual’s sixth (6th) and eighteenth (18th) birthday.
DDD will contact the parents or legal guardian before the individual’s birthday. DDD will tell them the redetermination is coming. Their Support Coordinator will help with the process. DDD will then review the current records. If more information or documents are needed, DDD will contact the parents or legal guardian.
When a child turns six years old, DDD needs records to show their diagnosis and limitations. Some examples of records are medical and educational. A new application is not needed.
The parents or legal guardians only need to submit a new application before the individual turns 18. They can fill it out during a Person-Centered Service Planning meeting. The new application then goes to the Support Coordinator. DDD may need records to show the individual still has limitations. Some examples are medical, educational, and employment records.
The DDD Assistant Director can ask for a redetermination at any time. This can happen if someone no longer needs DDD services.
