Division of Developmental Disabilities
The Division of Developmental Disabilities (DDD) is making changes to better support members who are both Children’s Rehabilitative Services (CRS) and DDD Arizona Long Term Care System (ALTCS) eligible.
DDD has directly contracted with UnitedHealthcare Community Plan to continue to provide your services with CRS starting October 1, 2018. This means that you do not need to take any action. No changes to your services are expected. Your Support Coordinator will also be your primary point of contact for services through CRS.
DDD is here to help you. If you have any questions, please contact the DDD Customer Service Center at 1-844-770-9500.
