The Chronic Disease Self-Management Program (CDSMP) self-management workshops provide people who have ongoing health problems with information about managing those problems more effectively. Workshops are guided by trained leaders who live with chronic conditions and offered in community settings. The workshop is two and a half (2 ½) hours per week, for 6 weeks and cost varies by location.
Topics covered include:
Anyone with a chronic condition such as diabetes, arthritis, heart disease, asthma, multiple sclerosis, or any other on-going health problem should participate
Caregivers or family members who serve as caregivers are welcomed.
