AzEIP Family Survey
A family survey is given to all families who have been served by AzEIP for six months or more, as the child turns age three, or is exiting AzEIP for any reason. AzEIP relies on responses from families to meet federal reporting requirements. We are required to report the percentage of families who report early intervention services helped their family to:

Interagency Coordinating Council for Infants & Toddlers
The Interagency Coordinating Council (ICC) advises and assists the Arizona Early Intervention Program to promote and improve early intervention for families and professionals. To learn more, read the ICC Brochure
