All Qualified Vendors must complete a Credentialing Process before they can provide services. DDD regularly reviews its provider network across the state.
Before applying to become a Qualified Vendor, review these steps:
Visit the Arizona Corporation Commission website to learn the Ten Steps to Starting a Business (This does not apply if you plan to apply as a Sole Proprietor.)
Have an internal system to follow provider requirements, including:
Employee background checks
Staff certifications
How services are delivered
Documentation
Billing for services
Applicants intending to apply for nursing and/or occupational, physical, or speech therapy services must get a National Provider Identifier (NPI).
Applicants intending to apply for Nursing, Home Health Aid, and Licensed Health Aide services must get a Home Health Agency License with the Arizona Department of Health Services.
