Within a few days of applying for Unemployment Insurance Benefits (a process also referred to as filing an initial claim), you will receive a Wage Statement in the mail that lists wages reported to the Department during the quarter in which they were paid to you. Your Wage Statement will show the quarters in your Base Period and the total wages paid within the months in each quarter.
The Wage Statement will also show the weekly and a total benefit award you may receive if you meet all eligibility requirements during your benefit year. The benefit year is normally the Sunday of the week in which you file your initial claim for Unemployment Insurance benefits. Your benefit year ends 365 days later. Your benefit year ending date will appear on your Wage Statement (UB-107). If your wages did not meet the criteria to establish a monetary award (your UI claim is ineligible because you did not have sufficient wages in the current base period to qualify), the reason why you did not qualify will be shown on the Wage Statement. When you receive your Wage Statement, examine it carefully. It will include only those earnings of record under the Social Security number that you gave on your initial claim. If you find incorrect amounts, wages that were not earned by you or earnings that were omitted, you may file a Wage Protest. It is recommended that you file a wage protest within ten working days of the date shown on your Wage Statement. Receiving benefits to which you were not entitled, when based on wages that were reported incorrectly or not earned by you, is a felony.
If you receive benefits based on wages not earned by you, you will be liable for repayment and may have other penalties imposed. Continue to file weekly claims while any wage investigation action is pending.
If you don't receive a Wage Statement or other claim documents in the mail within seven days after filing your new application for UI benefits, email the AZ Unemployment Insurance Call Center.
