In the past, taxpayers who owed past due Unemployment Tax and Job Training Tax received separate statements for each of those taxes. That has changed with the new Unemployment/Job Training Tax Statement (form UC-145A), a single statement that lists all amounts of Unemployment Tax, Job Training Tax, and Special Assessment past due. Reimbursement employers will also receive statements with a revised formatting. New form UC-145B displays amounts due in a more straightforward manner than the old statement. (UC-145B Sample 226 KB PDF).
