Your information must be updated with DCSS before you may request a replacement Electronic Payment Card from Bank of America. Please contact DCSS to update your information first. We will then submit a request to have your information updated within Bank of America's systems. If you need to update your address or contact information (such as your telephone number or e-mail address), you may contact DCSS either through our website or by calling our Customer Service Department at 602-252-4045 or toll free at 1-800-882-4151.
In order to officially change your last name in our system, we must obtain a copy of your updated driver's license and a copy of the legal document changing your surname. You can mail this information with your request and ATLAS case number to DCSS, PO Box 40458, Phoenix, AZ 85067. Once this information has been received, your name will be updated in our system.
Once your information has been updated, you may contact Bank of America's Electronic Payment Card Department within 24 to 48 hours to request a new card by calling 1-855-847-2030.
If you have never had a card, you will need to fill out the Electronic Payment Authorization Form on this website and mail it into the address on the form.
You can also call our Interactive Voice Response (IVR) system at 602-252-4045 or 1-800-882-4151 and request an Electronic Payment form which will be sent to you by mail, or you can walk into the local child support office closest to you and pick up the form.
