Nutrition Assistance (NA) (formerly the Food Stamp Program) requires an eligibility interview (also called a fact-finding interview) to determine eligibility. You will need to bring several documents to the interview. At the end of your interview, we will let you know if we need any other information or proof to decide your case. We will also send you a letter asking for this information.
You can mail, fax or bring in copies of the requested information to our office. Please do not turn in original documents.
When will I know if I am approved?
A decision will be made by one of the following dates:
- Within seven days from the application date if you qualify for expedited benefits
- At the latest, 30 days from the application date
To serve you better and to make sure you get all the benefits you need, please:
- Give us all the information and proof as soon as possible
- Ask if you need help getting information, so we can assist you
When we make a decision on your case, we will mail you the results. You may also monitor the status of your application on-line at Health-e-Arizona Plus or MyFamilyBenefits.
What can I do if I disagree with the decision?
An appeal may be requested for any eligibility decision when an application is denied, when benefits are stopped, or when an application is not acted upon in a timely manner. Learn more about appeals.
