Home and Community Based Services (HCBS) provide help with daily activities. HCBS allow many individuals to remain in their own homes or live with their families. They reduce the need for institutional care.
Adult day care, adult day health care, home delivered meals and meals at senior centers, home health aide, housekeeping, personal care, respite care, transportation and visiting nurse.
Eligible individuals are:
- 60 years of age or older, or
- 18 - 60 years of age with a disability,
- AND functionally impaired and unable to perform activities of daily living. This includes eating, bathing, dressing, grooming, using the bathroomand mobility (for example, walkingor moving from a wheelchair to a bed).
Applying for Services:
The services are provided through partnerships between DES and Arizona's Area Agencies on Aging. Area Agencies on Aging coordinate services, and their case managers determine eligibility. They can also answer many of your questions about long term services and supports. Select your regional Area Agency on Aging.
There are no required fees, but voluntary contributions help sustain services. A case manager will answer your questions about contributions to the cost of services.
