File a Complaint on a DES Child Care Provider

Filing a complaint is a responsible attempt to protect a child or children.

If you have knowledge of, or concerns about, a suspected violation committed by a DES Child Care Provider, please file a complaint.

The complaint is valid if you suspect a past or ongoing:

  • violation of Arizona state rules and/or statutes
  • child abuse or neglect

Report a Child Care Provider

  1. Be prepared to provide the following information:
    • Complainant’s contact information
    • Child Care Provider/Facility’s name, license number, phone number, address (or cross streets)
    • Description of alleged incident or violation
  2. Submit a Child Care Provider Complaint on-line or over the phone:
    • Please visit www.azccrr.com  and click on "Submit Provider Complaint"
    • Toll-Free: 1-800-308-9000 (Monday-Friday 8:00 a.m. -5:00 p.m.)

After You File the Complaint

The complaint will be sent to the appropriate regulating agency (Arizona Department of Health Services, Arizona Department of Economic Security, or Arizona Deparment Of Child Safety) or law enforcement. Investigation steps may include some or all of these:

  • Review of records
  • Interviews with witnesses, staff and, if appropriate, child(ren)
  • Obtain/review reports from other agencies
  • Complete compliance inspection

Frequently Asked Questions

What if I don’t want to give my name or want to remain anonymous?

  • Insufficient information will not allow for a complete investigation.
  • The person reporting complaint (complainant) will be unable to receive feedback.
  • Arizona statute requires a name, unless it could result in harm for the complainant.

    • Click on “Advanced Search Criteria” 
    • Enter Provider Name in text box
    • Click on Provider Details
