1099-G Tax Forms

1099-G tax forms for 2025 have been issued and are available in CACTUS. DES has created a short video that walks through exactly how to find your 1099-G tax form in CACTUS.

In the video, you will learn how to:

Locate and view your 1099-G tax form - click “View”.
Dispute an issue regarding the information on your 1099-G.

When you click on “View”, your 1099-G form will be available in a new browser window. You can also locate your form in your message center or locate under your Unemployment Services widget.

You can access CACTUS at uibenefits.az.gov.

If you requested mailed delivery, your 1099-G will be sent to the mailing address on file. Even if you receive a paper copy, your form will also be available electronically in CACTUS.
