Change Personal Information for Unemployment Benefits

To change your name, address, or phone number:

  1. Download and complete the Personal Information Change Request form.
  2. Fax or mail the form to the number or address listed on the form.

For tax purposes, keep your address and personal information up to date. This is important even if you are not currently filing claims.

You can also update your address by calling the AZ Unemployment Insurance Call Center. Select Option 4.
