Q: Is the new Focus application the only way that Incident Reports can be submitted?
No. The online application is an enhancement that provides vendors another method to submit incident reports. This change will streamline the process for submitting IRs including fields for all required information. Accurately completing these fields will reduce the need to provide additional information about the incident that may be omitted through the existing email or telephone submission processes.
Q: Can I print or download a copy of the Incident Report for my records after submitting it through the application?
At this time there is no way to print or download a copy of the incident report once it has been submitted. The user can take a screenshot(s) of the incident report in the application PRIOR TO SUBMITTING in order to have a copy to retain.
Q: Can a vendor upload a document into the application rather than re-entering all of the data?
At this time there is no way for a vendor to upload a document into the online application.The application is designed with specific questions as well as conditional questions that change based on data entered.
Q: How do staff who currently do not have access to Focus get access to this online application?
The vendor’s Focus administrator can grant access to any application in Focus to users that will need access. New users can create a Focus account by visiting https://ddd.azdes.gov/organization/ddd/focusdd/frm_login.aspx and then selecting “Signup As a New User.” After they create an account, they can request access to a vendor account following the instructions in Chapter 11 of the DDD QVA User Guide and Manual. The vendor’s Focus administrator can follow the instructions found in Chapters 12 and 15 to grant those users access to the Incident Reporting application.
Q: How do I add the Incident Reporting application to Focus?
When the application goes live on July 1, 2020, it will automatically be added to each vendor’s Focus administrator account.
Q: Can we submit incident reports for both standard and serious incidents through the Focus application?
Yes, any type of incident can be submitted through the Focus application.
Q: Does a written report still need to be completed and retained in the member files if the IR is submitted through the Focus application?
Yes, a written report still must be completed and retained in the member’s file as outlined in the Qualified Vendor Agreement Service Specifications Section 5.10.2.7.
