When using the AHCCCS Website or Medifax, consumers will see DES/DDD/LTC to identify your assigned health plan. However, AHCCCS cannot tell which actual health plan you are assigned because this Division assigns member health plans. AHCCCS considers this Division as your health plan, so they have no knowledge of what health plan you have chosen. To identify the health plan you have chosen, please call:
